First Visit Checklist

• Completed registration forms, sent via text

• Insurance card

• Medical records or, for newborns, hospital discharge information

• List of any current medications

 

We want your child to enjoy getting to know our physicians and nurses and be comfortable at all times. A pleasant, comfortable first visit builds trust and helps put your child at ease during future visits! We will explain everything in detail and answer any questions.

 

We believe that quality care begins with open communication. Our physicians and nurses are experienced in dealing with children and can explain treatment in a positive and pleasant manner. We also promise to speak candidly with you about our diagnosis and recommendations, any alternative treatments, and any symptoms present that may require further treatment.

 

In order to deliver the best possible care, we expect you to provide us with your medical history, as well as any care you receive outside our practice, including current medications, recent test results, visits to other doctors, health care providers, behavioral health services, hospitalizations and emergency department visits.
